  • Proposals must be previously unpublished material or literature reviews.

  • There may be only one person and one proposal per form. To submit more than one proposal, complete and submit this form, reload the page and repeat.

  • Co-authors may be listed. However, only the person submitting the Call for Papers Agreement will be considered as a presenter for this proposal. Upon invitation, only the person submitting the Agreement portion of this Application may make the presentation. He/she will be listed in the program as the sole presenter.

  • Supplemental materials will not be considered or reviewed.

    AGREEMENT
    If the above proposal is accepted for the 2009 AAV Annual Conference & Expo, I agree:

    1. To personally present/teach the session outlined above;
    2. To provide a manuscript in accordance with the Guidelines and Instructions to Authors for the 2009 Proceedings;
    3. To meet the March and April deadlines for submission of the manuscript and Summary Style manuscript, respectively, to the AAV Conference Office, recognizing that if my manuscript is not received by the receipt deadline (to be determined), AAV may cancel my presentation for the 2009 Annual Conference & Expo;
    4. That the AEMV or AAV Conference Committee has the right to cancel my presentation and publication of my manuscript in the Proceedings if, upon medical review, my manuscript is determined to be unacceptable for any reason;
    5. That it is my responsibility to ensure receipt of this application in the Conference Office by the December 1, 2008 deadline. The Conference Office will send a receipt within 7-10 days of receipt of this application.
    6. To accept compensation according to the AAV Speaker Compensation Plan; and
    7. That my signature below represents and warrants that I created and am the exclusive owner of all rights, title and interest in the Work proposed, including the copyright therefore, and that I have the full power and authority to enter into this Agreement.
